i’m not sure how obesity affects body composition/body build (it makes you look bigger but in a bad way, i guess?) but obesity makes it harder for your heart to pump blood to all parts of your body because it has to pump blood to a larger amount of body mass.
Since the heart pumps blood to the lungs to get oxygen into the blood, the lungs have to do more work too (in trying to get oxygen into the blood).
Simply, extra body weight (more than the appropriate weight, which differs from person to person depending on stuff like how big your bones are, activity, etc.) puts extra strain on the heart and lungs to get oxygenated blood to your whole body.
